Collect and Verify Health Information

This user guide is for the following MyJobCorps user roles: Admissions Representative (AR), OA Manager (OAM)

Overview

The Health tab in Gateway is where you manage applicant health information needed to complete Job Corps admissions processing, prior to QA Review. It serves as a hub where you:

  • View applicant health information organized in 4 sections: General Health, Medical Health, Oral Health, and Behavioral & Emotional Health.
  • Verify the completeness of an applicant's Health Questionnaire (Form ETA 653).
  • Confirm if supporting documents such as signatures, chronic care forms, and immunization records, are required.
  • Update health information when necessary. 

Applicants must be confirmed eligible, or conditionally eligible, before collecting health information.

Managing health information is a critical step in the admissions process for applicants in Gateway. The following guide explains how you can verify, edit, upload, and manage the Health Questionnaire (ETA 653) in the Gateway.

Review Health Questionnaire

Before any health review or verification can begin, applicants must sign and submit their Health Questionnaire in Portal- after they sign and submit their application; or you can also manually collect and enter health information within the Health tab or upload the Health Questionnaire (ETA 653) if needed.

Use the Health Questionnaire Submitted column on the Applicant tab of your Home view or Search view to monitor this action. 

After confirming an applicant is preliminarily eligible for Job Corps, the Health tab and tile unlock. You will be able to review the applicant’s health information or manually enter and upload the applicant’s health details- as well as supporting documents.

  1. Select the tabs from the left panel of the Health tab to confirm all fields have answers. 
  2. Gateway will identify if additional actions are required based on the applicant’s answers and age.

Collect Supporting Documents, if needed

If additional supporting documents are required, you can download these documents from the Forms tab or from the Health tab's automated alert to a needed Chronic Care Form. This includes Chronic Care Management Plans, immunization records, and/or other Provider documents.

  1. Provide the form to the applicant outside of the system.
  2. When completed and returned, upload the supporting documents to the Documents tab. Be sure to upload the documents using the appropriate document type from the drop-down menu.

Collect Parent/Guardian Signatures, if needed

If the applicant is a minor, parent/legal guardian signature is required. You will need to collect their signature outside of MyJobCorps and upload the signed document selecting that it has Parent/Guardian signature.

  • Collect the parent/guardian signature outside of Gateway.
  • When completed and returned, upload the signed Health Questionnaire as a PDF to the Documents tab. Make sure to select Health Questionnaire (Form ETA 653) as the document type.
  • If additional forms were required that include applicant signature, confirm parent/guardian signature is also completed before uploading.

Verify Health Questionnaire

The Verify Health Questionnaire box is displayed on the right side of the Health tab.

Verification ensures the applicant's health information is accurate, complete, and signatures are valid.

  1. Complete the verification checklist.
    • Ensure all Health Questionnaire fields are filled out.
      Reminder: Disclosure of medical history is voluntary. Applicants are not required to provide an answer to the questions but do need to sign the form. Fields marked "Prefer not to answer" are considered complete responses. 
    • Confirm the Health Questionnaire includes the applicant's signature. 

Note: If the applicant is a minor, parent/legal guardian signature is required. 

  1. Once the questionnaire has been reviewed and the checklist is complete:
    1. Select the check box to confirm your review.
    2. Select the Verify Health Questionnaire button
    3. The status of the Health Questionnaire updates from Verification Required to Accepted in both the Health tab and Document Center.
  2. When uploading health forms in Gateway, verification is automatic.

Note: Use the Tool Tip by hovering over the question mark next to checklist items for clarification on specific requirements.

Update Submitted Health Questionnaire, If Needed

If updates to the Health Questionnaire are needed after it’s been submitted in Portal, work with your applicant to address the required changes using one of the following options:

Edit Health Questionnaire

If the Health Questionnaire requires edits after submission or you need to manually complete the applicant’s health information, Admissions staff can do so in Gateway using the Edit Health Questionnaire action. 

Reminder: You must be assigned to the case to make edits.

  1. From the Health tab, select the Edit Health Questionnaire button from the top-right corner of the page or from the Actions Menu dropdown.
  2. Make the necessary changes or provide required information in the Health Questionnaire.

  3. Select Save and Close to finalize and save your edits.
    • Any previous entries will be overwritten with the updated responses when saving.

If changes affect the signatures or require the applicant to resubmit the form, you can unlock the Health Questionnaire using the Unlock Health Questionnaire for edits feature.

Unlock Health Questionnaire

Have the applicant complete edits in Portal by unlocking Health Questionnaire in Gateway.

  1. From anywhere in the case, select the Actions menu.
  2. Select Unlock Health Questionnaire for edits and confirm the action in the popup.

  3. Notify the applicant to update to complete updates in Portal and resubmit the questionnaire.

After the applicant signs and resubmits their changes, the updates will display in Gateway. The updates will overwrite any previous information provided.
