Are there any other forms that may be required for applicants to determine eligibility?

Yes, certain forms are required based on the personal circumstances of the applicant and information provided in their application. Admissions staff are responsible for determining when these forms are needed. These forms require signatures that must be obtained outside of the MyJobCorps system by Admissions staff:

  • Record Release Authorization: Admissions staff must use this form to request educational records if the applicant can’t provide the documents themselves.
  • Statement of Support: Required if the applicant meets low-income criterion 3 based on claiming no earned income. Admissions staff must get this form signed by the person financially supporting the applicant.
  • Procedures for Providing Reasonable Accommodation, Reasonable Modification in Policies, Practices, or Procedures and Auxiliary Aids and Services During Admissions: Required for applicants who request reasonable accommodations for disabilities or other needs.

Admissions staff should upload these signed forms to Gateway when needed.
