The following agreements and forms are signed by an applicant in Portal when they submit their application. These forms are required by the PRH Chapter 1 to determine eligibility for Job Corps. After they submit their application, applicants are prompted to complete and sign the Health Questionnaire in Portal.
If the applicant is an unemancipated minor, you will need to obtain their parent/guardian signature outside of MyJobCorps. After the applicant signs the forms in MyJobCorps, download the signed form from either the Application or Documents tab in Gateway and send via email using Adobe sign to the parent/guardian.
