QHPs complete the review of disclosed documentation in Health E-Folder.
Once the QHP makes a determination on enrollment, any documentation or assessments completed or any additional documentation obtained by the QHP must be uploaded to Gateway by the Health and Wellness Reviewer (HWR) (i.e., Forms 2-04 and 2-05, etc.). Then the HWR completes the Health Recommendation in Gateway, selecting Cleared for Enrollment to have the Records Manager (CRM) move forward with planning for arrival, or Request Regional Review to submit the applicant file for regional review dependent upon the QHP’s determination on enrollment.
